My3k Application My3k is an application to assist in managing users to perform health self-assessment. My3k help Universiti Malaysia Terengganu’s (UMT) management to monitor user’s health condition.\r\nMy3k is owned and operated by the Universiti Malaysia Terengganu’s.<\/p>\r\n

Personal Information<\/p>\r\n

The app will not record user’s Personal Data except with the permission and voluntarily provided by the user. Information collected are used for monitoring and enforcement purposes by UMT \u00a0in dealing with the COVID-19 pandemic. This information is not shared with other organizations for other purposes unless specifically stated.<\/p>\r\n

Data Collected<\/p>\r\n

This app collects data with the consent of the user and is done voluntarily through, but not limited to, registration process, adding dependent information, answering health assessments, checking in at the premises using a QR code scan and contacting app support via email or technical support form.Data are also collected automatically \u2013 for example, location data from check-in via scanning of QR code and your health risk status resulted from answering the health assessment. Location data will only be collected once the permission to access device location services is granted and allowed by the user. Data collected varies according to the purpose (based on the use of functions \/ modules of the app used by the user), the Personal Data that are usually collected are as follows:<\/p>\r\n

User Registration\r\nName\r\nAddress\r\nIdentity Card Number \/ Passport Number\r\nPhone number\r\nCheck-in\r\nName Location<\/p>\r\n

The purpose of data collection is as follows:<\/strong><\/p>\r\n\r\n

• To assess your risk of being infected by COVID-19 based on the information you provide on your health condition and history of accessing location in campus,.<\/li>\r\n \t
• To communicate with you in case your condition requires a follow up from healthcare personnel.<\/li>\r\n \t
• To respond to you about any comment or enquiry you have submitted.<\/li>\r\n \t
• To suggest the nearest medical\/screening facility to your location.<\/li>\r\n \t
• To ensure the compliance to all Movement Control Order (MCO) rules and Standard Operating Procedures (SOP) issued under the Prevention and Control of Infectious Diseases Act 1988.<\/li>\r\n<\/ul>\r\n \r\n

  Your personal information might be shared with enforcement authority for follow-up and resolution of any complaints you submitted via this App.\r\nThe Personal Data collected will not be used for any purpose other than those mentioned above, unless if required in order to comply with any legal obligation.\r\nNo Personal Data collected by this App will be disclosed to any third party or transferred to a place outside of Campus for commercial purposes.<\/p>\r\n

  Effect If Personal Data Is Not Sufficient\r\n<\/strong>If the Personal Data provided is insufficient or unsatisfactory, then the User’s application or request for any of the above purposes may not be accepted; or action taken or use of the services offered by the App may be rejected or affected.<\/p>\r\n

  Data Confidentiality\r\n<\/strong>Personal Data collected by the App will be kept confidential in accordance with this Privacy Policy in accordance with any applicable laws which may take effect from time to time.<\/p>\r\n

  Anonymised data\r\n<\/strong>This App collects anonymised data about your device and OS (e.g. device model, OS version) to help us improve the App and provide a better user experience.<\/p>\r\n
